本研究では、脳卒中患者の家族介護者が、急性期・回復期・維持期の経過において、直面する困難な状況にどのように対処し家族の力（Mastery）を獲得していくのかを縦断的調査により明らかにする。特に家族の関係性、生活の変化に着目する。具体的な研究目標は、①脳卒中患者の家族介護者は、急性期から維持期において、どの時期にどのような課題に直面するのか、困難な状況下でMasteryを獲得するプロセスを明らかにする、②①のプロセスにおいて、医療者からの支援がどのように役立っているのかを明らかにする、の2点である。令和３～４年度は、脳卒中患者の家族へのインタビュー調査を実施する計画である。今年度は、回復期リハビリテーション病棟を有する施設へ研究依頼を進め、現在は４施設から研究協力を得ることができた。回復期リハビリテーション病棟入院中の脳卒中回復期にある患者の家族介護者を対象とし、研究協力依頼を行っている。６事例のインタビューを実施した。そのうち４事例は、脳卒中回復期（発症後３か月程度）、維持期（発症後７カ月以降）に２回のインタビューが実施でき、急性期から維持期への移行のなかで家族介護者から縦断的にデータを収集することができた。事例Ａの分析結果では、急性期では、医師との関係構築の困難が不安を増強させており、面会制限は、家族と医療者との関係性にも影響していることが明らかとなった。回復期では、失語による障害を筆談や交換日記により乗り越え、リハビリ状況から患者の回復の兆候を理解して、患者の望み通りの在宅介護が決定できたと考えられる。そして、維持期では在宅介護となり、経管栄養のケアや家事は家族メンバーの支援を受け、ストレス要因の影響を減らしていた状況を明らかにすることができた。今後は、事例ごとに質的内容分析をすすめるとともに、依頼施設を拡大し、データ収集を継続して実施する。
