项目摘要

下肢の血管障害であるPADを有する糖尿病患者では、軽度の足病変であっても創の治癒遅延から下肢切断に至ることも多く、救肢のために糖尿病患者の血管障害の予防は重要な課題となっている。足病変の予防を目指して実施されるフットケアには、足病変への理解を深め、フットケア行動の実施率を高める効果があるといわれているが、フットケアが血管障害の予防に効果があるのか検討した報告は見られていない。そこで、本研究の目的は、2型糖尿病患者を対象にフットケアプログラムを実施し、患者の身体の理解が深まり、セルフケア能力の向上、セルフケア行動の変化、PADのリスク因子の改善、足病変の有無や変化といった効果を長期的に検証していくことである。今年度の研究予定は、開発した「2型糖尿病患者の身体の理解をうながすフットケアプログラム」を実施し、その評価を行う計画であった。しかし、COVID-19の影響もあり、ケア提供者の確保、データ収集先の確保が困難となり、対象者を外来通院中の2型糖尿病患者ではなく、「まちの保健室」に参加している糖尿病を含め生活習する必要が生じた。修正した介入プログラムは、「動脈硬化の予防を目指したフットケアプログラム」とし、フットケアの介入を糸口として、身体への関心を高め、血管障害を予防していくことを促すという基本的な考え方はそのままとし、活動を一緒に実施したり、活動計を装着し活動と休息（睡眠）の状況を参加者が捉えられるような内容を追加した。介入は7か月間に4回の介入を実施、介入開始と2回目、3回目は2か月後に、4回目は3か月後に実施した。ケアの評価はベースラインと4回目の介入終了時に、①身体の変化（体重、血圧、動脈硬化度、握力、2ステップ値）、②生活の変化（歩数、睡眠時間や睡眠効率）、③セルフケア行動（SDSCAと記述データ）、④セルフケア能力（SCAQ）を測定している。
