项目摘要

本研究の目的は、化学療法を受ける乳がん患者の倦怠感セルフマネジメントを支援するハイブリッドケアプログラムを開発し、その有効性を評価することである。令和4年度は、以下2つの内容を並行して実施した。①対照群（パンフレットと記録用ダイアリーを使用した従来型のケア）のデータ収集乳がんの術前／術後補助化学療法を受ける患者9名に対して、計2回のケアプログラムを実施し、介入前と介入後にデータを収集した。収集したデータは、プロセス評価（インタビューによる認知と行動の変化、自己効力感尺度による対処能力の向上）とアウトカム評価（多次元倦怠感尺度CFSによる「倦怠感の緩和」、QOL-ACDによる「QOL向上」、活動量計を用いた歩数の測定による「活動量の維持」）である。介入後は、効力感が有意に向上し、「歩数を確認しながら歩いた」など生活での取り組み内容からも対処能力の向上が示唆された。また、介入後のQOL得点が有意に上昇したことからQOL改善を認め、活動量も維持されていた。一方で、倦怠感は介入後に有意に得点が上昇しており、その要因として化学療法の影響が蓄積していることが推察された。②ケアプログラムで使用するスマートデバイスのプログラミング「倦怠感について知る」「体調の波をつかむ」「対処法を見出す」「活動をひろげる」の4つの教材コンテンツと、「倦怠感」「効力感」の2つのセルフモニタリングツールを作成した。外来や化学療法室の待ち時間内での実施を想定し、教材およびセルフモニタリングツールは1つにつき3分程度、全てのプログラムに要する時間を20分程度でプログラミングした。最終確認を残し概ね完成した。
