项目摘要

本研究は,高齢者が罹患する可能性が高い誤嚥性肺炎に影響を与えるオーラルフレイルのうち，機能低下レベルであるとみなされる第1レベルから第3レベルまでを対象とし，地域在宅高齢者に対する簡便かつ効果的なオーラルフレイルへの予防的看護介入プログラムの開発を目的としている。令和4年度は，昨年度に引き続き，オーラルフレイル予防に対応している専門家（医師，歯科医，歯科衛生士，保健師，訪問看護師，呼吸サポートチームに属する専門看護師，認定看護師，理学療法士等）を対象に実際の活動内容とその評価方法についてのインタビュー調査を行った。インタビューの結果，1）地域包括支援センターの保健師，歯科医師，歯科衛生士，ケアマネジャー，言語聴覚士等らを中心に高齢者の通いの場を活用したフレイル予防の普及促進，具体的には自主活動グループや介護予防教室等での口腔の講話や体操等が行われていること，2）評価については高齢者のアセスメント票はあるものの，口腔に特化した評価指標等の設定が難しく，評価が困難であったり，できていない現状が多く語られた。3）高齢者自身がオーラルフレイルを予防するために必要なオーラルヘルスリテラシーとして，自身の口腔内の変化として「歯がぐらぐらする・歯ぐきから出血する」といった歯周病に関連した変化を察知できる力，「むせる・飲み込みにくい」といった嚥下に関連した変化を察知できる力，入れ歯の変調を察知できる力，口腔の変化に気付いた際に歯科に受信できる行動力等がコアとなることが明らかとなった。さらに今後も県内外の歯科医師，歯科衛生士，行政保健師にも協力を得ることが可能となったため，引き続きインタビュー調査を行い，質問紙調査に向けた質問項目の精錬に努める予定である。
