项目摘要

「高齢透析患者の透析開始時における意思決定と将来についての思い」を2022年12月に行われた日本看護科学学会のポスターセッションで発表を行った。対象者は、7名（男性6名、女性1名）で平均年齢は80.8±2.1歳であった。高齢患者は、いずれ《透析となる予告》を受けていた。治療選択肢についての説明はほとんどなく、《当然のような流れ》で透析導入された。その流れに対して《長く生きてきた結果》として受け止めたり、《限界まで我慢をする》ことで《導入への抵抗》を意思表示する者もいた。そして過去の自分の不摂生な生活に《後悔》しながらも《高齢の配偶者からの後押し》もあって《生きるための覚悟》をもって導入に踏み切っていた。しかし、高齢患者は、医師や看護師から透析についての理解度を確認されないまま《不十分な理解》で透析を導入していた。透析導入後、高齢患者は、透析後の強い倦怠感や体の衰えから《老化の自覚》をしていた。それで《健康的な生活の維持》を心掛けていたが、透析の知識が十分ではないため、《不十分な自己管理》となっていた。将来は、このままの状態が続き、楽に死ねたらいいという《楽観的な最期》を願っていたり、いつか透析をやめたいという《離脱願望》をもって透析生活を送っていた。この結果を今年度、論文投稿に向けて執筆をしていく。当初の計画は、透析導入前の高齢腎不全患者にフレイル予防の介入を行う予定であったが、コロナ禍の影響で研究の方向を変更せざるを得ず、75歳以上の透析患者のフレイル予防について現在取り組んでいる。透析中に運動あるいは認知トレーニングの介入を行い、身体機能と認知機能への効果を測定する。75歳以上の高齢透析患者であっても透析中の介入がフレイル予防および身体機能維持に効果的かを検討し、高齢透析患者の生活への満足につながるのかをQOL尺度で評価する。
