本研究の目的は、難病患者が生活困窮に陥らないように、発病時から就労と社会的孤立予防（以下、孤立予防と略す）の視点を持つ①伴走型就労・孤立予防メンバーとその役割を明確にし、保健師が②伴走型就労・孤立予防チームを構築するプロセスを明らかにすることである。2022年度は、就労支援経験のある保健師と相談員、難病患者を対象に行動結果面接を実施し、①支援の目的と行動、②就労・孤立予防チームの成功事例・失敗事例からチーム構築の課題と阻害要因を明らかにすることを目的とした。新型コロナウイルス感染禍にあり、難病患者への対面面接調査は実施できなかった。現在は、逐語録のデータを分析している段階である。生活困窮予防においては、難病の進行はある程度予測可能であり、発病時から就労と孤立予防の視点で早期的、個別的、包括的、持続的な人生支援が求められる。今年度の調査では、難病相談支援センター相談員とハローワーク難病就労サポーターの支援開始から支援チーム作りの過程及び多職種連携・協働の具体例を収集することができた。相談員は最初に支援に必要なチーム構成員を明確にし、課題を提示することによって患者を含むチームを形成、チームとして機能させていた。一方で、相談員の支援は就労困難な局面から始まるため、発症時から相談につながるまでの期間についての実態解明が課題である。今後は、就労支援対象となった難病患者ごとに相談支援者によるグループインタビュー調査を検討する。

In the future, we will discuss the investigation of support for patients with difficult diseases.
