项目摘要

本研究は、農村地域における認知症高齢者が農作業を行うことで、自身の生きがいや身体的側面における筋力にどのように影響するのか明らかにすることである。さらに、介護職員の介護負担感の変化を把握し、農村地域における認知症高齢者に対する農福連携のプログラムを開発することである。本年度では、農繁期・農閑期における認知症高齢者の下肢筋力の変化について明らかにした。研究方法は、岩手県NPO法人施設利用者で施設内の農作業に参加している認知症高齢者8名を対象とした。調査期間は、令和4年5月～令和5年4月で2か月～3か月に1回の間隔で、等尺性筋力ミュースタF-2により、膝関節伸展筋力測定を実施した（R4年5月,8月,10月,R5年1月,4月)。1回の測定は、左右各々3回測定し平均値を求めた。分析方法は、農繁期・農閑期の測定結果の結果を比較し変化を見た。統計解析は、JMP12.2（SASinstitute）を用い、「対応のあるT検定」を行い有意水準は5％未満とした。その結果、p=0.0125で有意な差がみられ、農閑期（4・5回目）の方が筋力が低くなっていた。今回、福祉施設の認知症高齢者の農繁期における下肢の筋力測定を行った。農繁期は、野菜の収穫や草取りなど、支援員の指導の下に行っている。中高齢期の身体活動や運動習慣の保有は、認知症やアルツハイマー型認知症発症率の低下との関連が示唆されている。また、運動介入はすでに発症したアルツハイマー型認知症者の認知機能を改善する効果も報告されている。農作業や草取りは畑に出て全身で行う作業であるため、継続的に作業を行うことが筋力低下の予防になっているのではないかと考える。施設利用者の方々は、ほとんどが地元の方で農家も多い。以前より慣れ親しんでいる農作業を進めていくことは、認知症の進行をお緩やかにするなど期待が持てると推察する。
