项目摘要

本研究の目的は、尿失禁の治療の第一選択とされ、ガイドラインにおいても推奨グレードAとされる骨盤底筋トレーニングを、より効果的に行うための方法の開発である。従来からの骨盤底筋トレーニングの大きな問題は、トレーニング方法を理解することが難しく、指導および実施が困難であることである。そこで、骨盤底筋群と解剖学的に連結しており、収縮時に互いに影響しあう股関節周囲筋の随意収縮を利用し、骨盤底筋群の活動を惹起可能かを明らかにすることとした。また、骨盤底筋トレーニングは仰臥位で指導されることが多いが、尿失禁がおこる姿勢は主に抗重力位であるため、抗重力位での活動を解明するため、グラビティMRIを用いることとした。実験1として、まずは、健常女性を対象に、股関節周囲筋の収縮が骨盤底筋収縮に与える影響を明らかにするため、立位での股関節外旋筋収縮、股関節内転筋収縮を行った際の骨盤的筋収縮について、グラビティMRI撮像を実施した。MRI撮像の分析は、矢状面で、恥骨尾骨ラインに対する膀胱頸部の位置の変化と、冠状断で、尿生殖裂孔の幅を変化を測定した。また、被験者が骨盤底筋収縮が可能かどうかを確認するため、エコーを用いてい、骨盤底筋収縮をおこなったとき、股関節内転運動をおこなったとき、股関節外旋運動を行った時の膀胱底挙上量を測定し、被験者の主観的な理解については、アンケートを実施した。エコーでの膀胱底挙上量について結果、未経産女性においては股関節姿位による有意差はなく、個人によるばらつきが大きい結果となった）。グラビティMRIを用いた膀胱頸部の位置については、股関節外旋位で股関節中間位に比べ有意に膀胱頸部の位置が挙上していた。また、尿生殖裂孔の幅においては、股関節内転位、股関節外旋位ともに、中間位より幅が狭小化しており、骨盤底筋収縮が無意識化で得られていた可能性が示唆された。
