この研究の目的は、進行した脊柱変性疾患に対する脊椎長範囲固定後の日常生活動作（ADL）制限およびその動作時の下肢関節運動や殿筋・下肢近位筋の活動パターンの変化を解析し、術後のリハビリテーション治療や姿勢・生活指導にフィードバックしていくことである。脊柱変形に対する手術が普及し、良好な疼痛軽減や脊柱アライメント獲得が可能となったが、一方で脊椎固定による可動性低下によって特に体幹前屈動作を伴うADLには影響がでやすい。基本的ADLにおいて、下肢の可動域や筋活動パターンの変化などが生じ、局所的な負荷の増大による新たな障害発生などが危惧される。よって、脊椎長範囲固定後の患者に対して、3次元動作解析と表面筋電計を使用した基本的ADL（歩行、立ち座り、段差昇降など）に関する動作および筋活動解析を行い、健常者および術前のデータとの相違点を検証することで、術後のリハビリテーション時の運動指導や姿勢・生活指導などの患者教育に応用できると考えている。高齢者に限定した症例数確保が難しいため、本年度より若年～中年者の症例も含めて検討を行っている。解析できたデータからは、術前は基本的ADLを行う際には、どの年代も凸側優位に背筋筋活動が高く、特に中年～若年者でその傾向が強かった。動作の中で、前屈や立ち座りの際には、左右差が大きかった。術後は、特に高齢者で股関節の可動域が増大しかつ殿筋の筋活動の増大を認め、これは脊柱固定後の代償性変化と考えられた。この結果より、術前からの体幹・股関節周囲筋の評価や運動療法が重要であると共に、術後のADLにおいても股関節およびその周囲筋に負荷が増大しないようなリハビリテーション治療や生活指導を行うことは、新たな障害発生の予防につながると考えられる。今後、症例数を増やして研究をすすめるとともに、バランス評価なども追加して行い、転倒予防の対策としても検証を行う予定である。

The purpose of this study is to analyze the changes in the movement of lower limb joints, muscles and proximal muscles after spinal cord fixation, treatment, posture and life guidance for spinal cord disorders. Spine reconstruction surgery is popular, good, pain reduction, spine recovery, and possible, one side spine fixation, low mobility, and special trunk flexion movements associated with ADL. Basic ADL, lower limb mobility, muscle activity, load increase, new damage, danger 3-D motion analysis and surface electromyography are used in patients after spinal length fixation. Basic ADL (walking, standing, and elevation) is used in motion analysis and muscle activity analysis. For healthy patients, motion guidance, posture guidance, and life guidance are used in patient education before and after surgery. The number of cases of high-aged people is limited to ensure that the number of cases of middle-aged people in this year is limited. Analysis: Before surgery, basic ADL activity is higher, middle age is higher, and younger is higher. The action is in the middle, the front is bent, the seat is in the middle, the left and right are different. After surgery, the range of motion of the femoral joint was increased in patients with high spine, and the compensatory change after spinal fixation was observed. The results showed that exercise therapy was important before operation, ADL was important after operation, and the load on femoral joint and peripheral tendon increased after operation. In the future, the number of cases should be increased, and the evaluation should be carried out in advance.
