项目摘要

小児肥満（症）患者あるいは成人に達した小児肥満症経験者について、レプチンメラノコルチン摂食抑制系に関わる遺伝子群の病的バリアントを検出する率を研究する計画である。レプチン遺伝子、レプチン受容体遺伝子、プロオピオメラノコルチン（POMC）遺伝子、MC4R遺伝子などを標的として次世代シーケンサーを用いて、遺伝子の塩基配列を解析する。データベースと照合し既知の病的バリアントの検出する。病的意義が不明なバリアントが検出された場合はPolyPhen2などのタンパク質の機能予測モデルプログラムを利用し、病的意義を考察する。2022年度は京都府立医科大学医学倫理審査委員会における審査にて研究許可の承認を得た。2022年10月から研究対象者のエントリーを開始した。2023年3月末時点で12例の小児期に発症した肥満症の患者をエントリーした。現在、本研究コホートの参加者について末梢血からテンプレートDNAを抽出し、次世代シーケンサーを用いて遺伝子を解析中である。また、次年度以降も研究対象者を増やして、解析を進める予定である。別の研究コホートとして甲状腺ホルモン２型脱ヨード酵素遺伝子の多型、rs225014 (DIO2, Thr92Ala)について検討した。Ala/Ala多型は小児肥満者に占める割合が15.1%、非肥満者の6.3%と比べてオッズ比3.393 (p=0.003)と高かった。多変量解析でもオッズ比3.757[95%CI,1.645-8.580; p=0.002]であった。rs225015は小児肥満のリスクを高める遺伝子多型である可能性が示唆された。
