終末期がん患者ではランダム化比較試験等の実施が困難であり、またランダム化比較試験の集団と比べると、実臨床の集団は脆弱性が極めて高く、介入試験の結果をそのまま臨床応用できないという問題点がある。本研究は、緩和治療のレジストリ研究であり、終末期がん患者の難治性症状に対する薬物療法のリアルワールドでの安全性・有効性を明らかにすることを目的とした研究である。対象となる難治性症状および症状緩和治療について、あらかじめ研究協力者と協議してリアルワールドでの症例数、治療内容、アウトカム指標を整理したうえで、webベースのレジストリを用いて研究参加施設から臨床データを収集し、治療の安全性・有効性を明らかにするという手順で研究を進める。2022年度は、せん妄治療に関する総説の作成し英文誌に投稿（Maeda I et al. J Pain Symptom Manage. 2023 Jan;65(1):e81-e85.）を行ったこと、研究協力者と共同して、せん妄(Abe A, Maeda I et al. Jpn J Clin Oncol. 2023 Mar 30;53(4):321-326., Inoue S, Maeda I Acta Med Okayama. 2022 Apr;76(2):195-202. 他)、死前喘鳴（Yamaguchi T, Maeda I. Jpn J Clin Oncol. 2022 Jul 8;52(7):774-778., Yamaguchi T, Maeda I. BMJ Support Palliat Care. 2022 Nov 10 ）に関する実臨床データをまとめて報告したことが主な成果である。引き続き、研究参加施設、データセンターと緊密な連携を取って、終末期がん患者に対する緩和治療のリアルワールドデータを得られる仕組みの構築を行っていく。

In the terminal stage, the patient has difficulty in implementing the comparative test, and the clinical group has high vulnerability. The results of the intervention test have problems in clinical use. This study aims to investigate the safety and effectiveness of palliative therapy in patients with refractory symptoms. For refractory symptoms and symptomatic palliative therapy, study collaborators, protocols, number of cases, treatment content, and indicators of failure are collated, and web sites are used to participate in research settings, clinical data collection, treatment safety, effectiveness, and research progress.
