膵体尾部原発の浸潤性膵管癌の手術症例（n=66）を後方視的に解析した．症例は男31例，女35例，年齢の中央値は72（最小値43，最大値83），術前化学療法施行例が12例，Stage Iが12例，Stage Ⅱが53例（ⅡAが24例，ⅡBが29例），StageⅢが1例， 病理組織学的腫瘍径の中央値は28mm（最小値2mm，最大値75mm），原発巣におけるリンパ管侵襲陽性例は47例，リンパ節転移陽性例は29例であった．術前化学療法施行例では癌のリンパ管侵襲陽性の比率が有意に低かった（P<0.05，Fisherの正確検定）．また，癌の膵前方組織浸潤，膵後方組織浸潤，リンパ節転移のある症例では癌のリンパ管侵襲陽性の比率が有意に高かった（P<0.05，Fisherの正確検定）．年齢や性別，病理組織学的腫瘍径，癌の脾動静脈浸潤，膵外神経叢浸潤，他臓器浸潤と癌のリンパ管侵襲との間には有意な関連性は見られなかった．

Surgical cases of invasive ductal carcinoma originating in the caudal region (n=66) were analyzed retrospectively. Among the patients, 31 were male and 35 female. The median age was 72 (minimum 43, maximum 83). The preoperative chemotherapy was performed in 12 cases. Stage I was performed in 12 cases. Stage II was performed in 53 cases (ⅡA in 24 cases, ⅡB in 29 cases). Stage III was performed in 1 case. Pathological histology showed that the median diameter of the tumor was 28mm (minimum 2mm, maximum 75mm). The positive rate of vascular invasion was significantly lower in patients treated with preoperative chemotherapy (P<0.05, Fisher's correct estimate). The positive rate of tumor invasion was significantly higher in all cases (P<0.05, Fisher's correct estimate). Age, sex, histologic diameter, splenic arteriovenous invasion, extra-nervous plexus invasion, organ invasion, and vascular invasion of cancer.
