HBV既往感染歴を有する造血器腫瘍治療後のHBV再活性化に対し、定期的なHBV DNAモニタリングに基づく核酸アナログ先制治療により関連する肝障害予防は可能になったが、核酸アナログの中止規準は確立されていない。我々は、名古屋市立大学病院にて2008年から2020年までに、HBV既往感染歴を有する造血器腫瘍患者27例のHBV再活性化後の保存血清を用いて、高感度HBコア関連抗原(iTACT-HBcrAg)を測定し、その有用性をレトロスペクティブに検討し、iTACT-HBcrAgは早期のHBV再活性化診断に役立つとともに、核酸アナログを安全に中止するためのサロゲートマーカーとなりうることを報告してきた（Hagiwara S, et al. Hepatol Res. 2022 Sep;52(9):745-753.）。さらに、名古屋市立大学関連病院4施設と多機関合同のレトロスペクティブ研究を実施し、2008年から2020年までにHBV再活性化と診断した44例について、iTACT-HBcrAgの意義を検討した。その結果、先行研究（名古屋市立大学単施設）と同様にiTACT-HBcrAgの有用性が検証された（Suzuki T, et al. J Gastroenterol. 2022 Jul;57(7):486-494.）。

HBV reactivation after treatment of hematopoietic tumors with prior HBV infection may be associated with periodic HBV DNA monitoring and prevention of liver damage associated with prior therapy. From 2008 to 2020, 27 patients with hematological lesions who had HBV infection were selected from Nagoya City University Hospital. The serum was stored after HBV reactivation. The serum was used as a medium and highly sensitive HB-related antigen.(iTACT-HBcrAg) assay and its usefulness in early HBV reactivation diagnosis are reported (Hagiwara S, et al. Hepatol Res. 2022 Sep;52(9):745-753.). In 2008, Nagoya City University-affiliated Hospital conducted a multi-agency contract for HBV reactivation and diagnosis in 44 cases, and the significance of iTACT-HBcrAg was discussed. Results, pilot study (Nagoya City University) and validation of iTACT-HBcrAg usefulness (Suzuki T, et al. J Gastroenterol. 2022 Jul;57(7):486-494.）。
