関節リウマチ（RA）患者においてヤヌスキナーゼ阻害薬（JAKIs）使用下の血管イベント（深部静脈血栓症、肺塞栓症、動脈血栓症、急性心筋梗塞、脳梗塞）のリスクを検討した。DPC病院のレセプトデータ／DPCデータを用いて、傷病名およびJAKIs、生物学的製剤（bDMARDs）、メトトレキサート（MTX）の処方データを用いて16歳以上のRA患者で新規に上記薬剤が処方された患者を同定した（n=53,448、MTX群n=37,926、bDMARDs群n=37,926、JAKI群n=4,416）。血管イベント全体の発生率 [95%信頼区間]（/1000人年）はMTX群では11.29 [10.61-12.01]、bDMARDs群では6.81 [6.20-7.46]、JAKI群では10.12 [7.85-12.86]だった。発生率比[95%信頼区間]は、1.49 [1.14-1.93]（JAKI群vs.bDMARDs群）、0.90 [0.70-1.16]（JAKI群vs. MTX群）だった。時間依存性Cox回帰分析を用いて患者の年齢、性別、併存症、既往症で調整し、JAKIの調整済みハザード比を算出したところ、1.52 [1.14-2.03]（vs. bDMARDs）、1.20 [0.49-2.85]（vs.MTX）だった。DPC病院のレセプトデータ／DPCデータを用いて、高安動脈炎（TAK）患者（n=952、0-19歳は63名、20－64シアは650名、65歳以上は241名）における治療内容等の診療実態を記述した。最も有病率が高かった併存症は高血圧だった。生物学的製剤が処方された患者の割合は全体では24.2％、19歳未満では55.6％、20－64歳では27.4％、65歳以上では7.5％と若年ほど高く、入院率は高齢になるほど高く感染症などの合併症による入院が増加していた。

Joint thrombosis (RA) patients are treated with JAKIs and vascular diseases (deep venous thrombosis, pulmonary embolism, arterial thrombosis, acute myocardial infarction, and stroke) are examined. DPC hospital's new prescription for RA patients aged 16 years and older, patient name, JAKIs, biological agents (bDMARDs), MTX (MTX), and the same prescription for RA patients (n=53,448, MTX group n= 37,926, bDMARDs group n = 37,926, JAKI group n=4,416). The overall vascular survival rate [95% confidence interval](/1000 person-years) was 11.29 [10.61-12.01] for MTX group, 6.81 [6.20-7.46] for bDMARDs group, and 10.12 [7.85-12.86] for JAKI group. The survival rate ratio [95% CI] was 1.49 [1.14-1.93](JAKI group vs. bDMARDs group), 0.90 [0.70-1.16](JAKI group vs. bDMARDs group). MTX group). Time-dependent Cox regression analysis was used to calculate the adjustment ratio for age, sex, comorbidities, and past conditions of patients, and the adjustment ratio for JAKI was 1.52 [1.14-2.03](vs. bDMARDs) and 1.20 [0.49-2.85](vs. MTX). DPC patients (n=952, 63 aged 0-19, 650 aged 20-64, and 241 aged 65 or older) were treated in the hospital. The highest incidence of complications is high blood pressure. The proportion of patients prescribed by the biological system decreased from 24.2% to 55.6% at the age of 19, from 27.4% at the age of 20-64, and from 7.5% at the age of 65 and older. The hospital admission rate increased from 10% to 10% at the age of 19, and from 10% to 10% at the age of 65.
