项目摘要

本研究の目的は、若年期におけるピロリ菌感染防止の胃がん予防への有効性について、経済面からの議論の根拠となる基礎データを構築することである。ピロリ菌感染の次世代への継承を防止するために若年期にピロリ菌感染検査及び除菌を行うことで期待される将来の胃がん罹患率の減少効果をモデル化し、経済評価を行う。2022年度は、日本人における生年別ピロリ菌感染率の推計値（Wang C, et al. Sci Rep. 2017）と地域がん登録・高精度地域実測値：がん罹患年次推移データ（1985年～2012年）及び人口動態統計がん死亡データをもとにシミュレーション解析により推計したピロリ菌感染の有無よる出生からある年齢に達するまでの胃がん累積罹患リスク（2021年度に論文発表、Kawai S, et al. Int J Cancer. 2022;150:18-27.）を用いて、ピロリ菌除菌した場合の胃がん予防効果について、15歳の仮想コホートに対してマルコフモデルを用いてピロリ菌除菌の有無による胃がん長期累積罹患リスクを男女別に求めた。さらに、実際に中高生を対象に行われている自治体のピロリ菌感染対策事業の経費実績を用いて85歳までの胃がん罹患者数を介入の有無により推計し、ピロリ菌除菌治療のみで胃がん罹患1件を予防するのに必要なコストを算出した。また、大規模コホート研究に参加した一般成人男女約5,000人を対象としてピロリ菌感染状況について検査結果と自己申告の間の一致率について検討し、論文投稿中である。
