项目摘要

新型コロナウイルス感染症の蔓延のため、過疎高齢化地域の住民参加型研究（CBPR）については準備状態のまま延期せざる負えない状況であった。そのため、研究計画を軌道修正し、昨年度に引き続き、高齢者施設の感染管理体制の実態と入所者ケアにおける職員の感染予防行動への影響要因について、山梨県と長野県に、宮崎県を加え３県で調査を実施した。研究協力の意向が確認できた高齢者施設を協力施設とし、そこに勤務している①感染対策担当者と②利用者ケアを実施している全職員のうち同意が得られた者を研究協力者とした。基礎情報および感染対策の状況については単純集計を行い、おむつ交換時における手指衛生と個人防護具の使用について、手指衛生は「利用者ごと」と「それ以外」、個人防護具の使用は「いつも使用」と「それ以外」に分け、職員の感染予防行動の実施と『行動意図』の影響要因について、χ2 検定を行い2群間の関係を比較検討した。3県の高齢者施設832施設中288施設(回収率34.6%)から返信があり、研究協力の意向は106施設(12.7%)から得られた。106施設へ質問紙調査を実施し、回収は96施設(回収率98.0%)、①感染対策担当者は96施設中94人(97.9％)、②利用者ケアを実施者は欠損回答を除いた596人(68.6％)を分析対象とした。協力が得られた高齢者施設は感染管理体制が整備されており、おむつ交換時は手袋・マスクを使用していたが、袖なしエプロンを使用している施設は50%みられた。高齢者施設職員の感染予防行動への意識は高かったが、介護職と看護職ではエプロン、マスク、ゴーグルに使用差があり、看護職の防護具着用が有意に高かった。感染予防行動を職業別に影響要因をみると、介護職は【主観的規範】が高く、看護職は【感染予防行動への態度】と【感染予防行動へのコントロール感】が高いことが明らかとなった。
