项目摘要

2022年度の目標は、従来のHospital Incident Command System ：HICSの成果と課題を明らかにし、協力病院におけるHICS導入後の準備状態やリスクを明らかにすることである。しかしながら、コロナ禍の影響が続き、大規模対面での訓練が中止され、小規模な訓練にならざるを得なかった。そこでHICSと同様に災害対応訓練として近年注目されているエマルゴを実施し、HICSとの違いを検討した。エマルゴは、エマルゴキットと登録されたインストラクターの下で参加者が研修を受講するものである。コロナ禍で大規模な対面訓練が難しい中、机上シミュレーションへの期待が高まり、2022年度研究協力病院では、エマルゴを実施した。傷病者の想定は、発災日24時間の来院患者を400名、6時間で93名が搬入されるというものであった。また、傷病者数の想定は現実的なものとなるように、過去の災害のデータを活用して設定した。今回の訓練を通して、待機ベッドの確保や入院調節ポストの確保が有効であることも示唆された。エマルゴは設定された病床数・増床数・職員数など限られた資源を用い，訓練上の時間経過に応じて変化するニーズに対応することが求められる。設定された対応法の妥当性の検討や避けられた合併症や死についても検討ができ、短い訓練の中で、現実的な課題が明らかになった。参加者の評価も高かったが、エマルゴはロイヤリティを支払う必要があるため，費用負担が課題となると考えられた。HICSもエマルゴも訓練の効果を高めるためには、リアリティのある設定と訓練されたファシリテーターが必要であるという点や一定のコストがかかる点が共通していた。
