项目摘要

地域包括ケアシステムの構築が求められ，自宅のみならず地域の多様な療養の場で最期まで生きることを支える体制が必要とされている。訪問看護を受けているがん患者は，末期と診断されていても，治療中止の見極めの難しさ等から治療を継続している場合があり，また，がん末期の症状コントロールも必要となるため，医療と切り離して生きることができない。地域で暮らすがん患者のエンドオブライフを充実させるうえでは，がん診療を受けている病院の外来看護師と訪問看護師との連携が重要となる。本研究課題では，自宅や居宅系施設など，住み慣れた地域で暮らすがん患者がより充実したエンドオブライフを送り，その人らしく生き抜くことが可能となることを目指すための，外来看護師と訪問看護師の連携モデルを構築する。本研究の目的は看護連携モデルを開発することであり，モデル開発はvan Meijelら2004）が提唱している根拠に基づく看護介入を開発する方法を参考に行う。開発手順には，①問題を記述する，②看護介入をデザインするために必要なものを積み上げる(文献検討，問題とニーズの分析，既存の実践の分析)，③看護介入をデザインする，④看護介入の妥当性を検証する，が含まれる。2022年度は，②看護介入をデザインするために必要なものを積み上げる(文献検討，問題とニーズの分析，既存の実践の分析)を実施した。文献検討では，「訪問看護を受けながら地域でエンドオブライフ期を過ごすがん患者の体験に関する統合的文献レビュー」を日本語・英語文献を対象に進め，研究結果の学会発表を行った。問題とニーズの分析として，「がん患者のエンドオブライフの充実に向けた訪問看護師と病院看護師の連携の実際と課題」を訪問看護師の立場から明らかにするべく，面接調査を行った。
