近年の医療技術の発展に伴い、急性期病院のみならず、療養施設、在宅においても医療機器は高度なものが広く使用されるようになってきている。そのような中で、治療や療養のために使用される医療機器、ケア用具の使用による圧迫創傷は医療関連機器圧迫創傷（MDRPU）、用具圧迫創傷（DRPU）として予防の必要性が叫ばれている。日本褥瘡学会における実態調査では、DVT予防用弾性ストッキングやNPPVマスクについて、チューブ類によるものが多いことが報告されている。研究代表者はこれまで弾性ストッキングによる圧迫創傷について研究を行ってきた。本研究の目的はこの知見を活かして汎用されている経鼻胃管チューブや膀胱留置カテーテルによる圧迫創傷の特徴を明らかにし、予防モデルを作成し機器や個々の特徴をふまえた予防ケアを構築することである。この研究により、医師、看護職が少ない療養生活、在宅の現場においてもMDRPUやDRPUの予防が行え、対象者が安全、安楽に生活できる。現在、Covit-19の感染拡大や施設への訪問制限などが大きく影響し、予定していた調査対象施設の選定や施設との調査の調整についてが進んでいないが、研究者の職場の移動も懸案し、調査対象の地域を石川県、岐阜県とした。調査協力者となる石川県下、岐阜県下の皮膚・排泄ケアの数名には内諾を得ており、調査用紙の作成を終え予備調査を進めている。実態やMDRPUの形状の特徴を明らかにするための画像提供を実施、一部依頼を進めている。MDRPU予防のための予防モデルの作成については、企業の協力を得て、チューブの特徴、固定の特徴を整理している。併せて教育ツール作成の素案を作成中である。今年度前半は実態とMDRPUの特徴をまとめることを早急に行う。後半は、研究協力者を小規模の病院や療養施設でも依頼をし予防モデルや教育ツールの意見を求め、評価、検証について取り組む。
