本研究の目的は，不育症女性が流死産から次子の子育てに至るまでのケア・ニーズを明らかにし，周産期の悲嘆過程に沿った継続したケアシステムを構築することである。本年度は育児中に流死産した母親の体験と上の子との関わりを明らかにした上で，母親への支援を検討することを目的とした。そのため，育児中に流死産した母親3名を対象とし，半構造的面接を実施した。インタビュー内容から逐語録を作成後，質的帰納的に分析した。その結果，育児中に流死産した母親は，流死産が上の子に与える影響を配慮して上の子と関わっていた。また，母子ともに悲嘆反応が生じている中で，母親は自身と上の子のグリーフワークの両方に取り組んでおり，上の子と共に悲嘆過程を歩んでいたが，その対応には不安や迷いが生じていた。そして，流死産後の母親へ生活面や精神面の支援ができる存在がいない場合には，母親は孤立し，育児そのものに支障がでることもあった。また，育児中であることの特徴として，ママ友との関わりに大きなストレスを感じていたことも明らかとなった。これらのことから，ケア提供者は，流死産を体験した子どもとの関わり方について情報を提供したり，不安や悩み等の相談窓口を知らせたりすることが必要であることが明らかとなった。さらに，利用可能なソーシャルサポートにつなぐなど地域と連携し継続した支援が求められていた。また，ママ友を含めた周囲の人々との関わり方に関する情報提供なども求められていた。流死産の経験から生活面・精神面を支え，母子ともに円滑な悲嘆過程を歩むことができるよう支援していく必要があることが示唆された。次年度は，臨床におけるグリーフケアの実態について全国調査を実施し，現状を明らかにしたうえで今後の課題を検討する。
