選定基準を満たしたスポーツ選手2名（剣道、バドミントン）に対して、多血小板血漿(PRP)を用いたアキレス腱縫合術を行った。直視下に2号ファイバーワイヤーを用いてアキレス腱を強固に縫合し、アキレス腱断裂部の腱の一部を採取し、組織学的に変性の有無を評価した。手術室にて26ml採血し、GPSⅢシステム（医療機器製造販売承認番号：22700BZX00420000）を使用してPRP約3mlをクリーンベンチ内で生成した。腱を縫合後にPRP3mlを縫合部に注入した。術後は2週間のギプス固定後に、病理組織にて高度の変性所見がないことを確認し、早期運動療法（早期荷重と早期可動域訓練）をおこなった。外来通院の上、臨床評価と画像評価を行った。臨床評価は主観的評価として痛み、歩行、階段昇降、スポーツ復帰時期をアンケート調査する。客観的評価としては下腿周長、関節可動域、片足爪先立ちが10回連続で可能かどうかを調査する。MRI所見（腱の形状、腱内の信号変化、腱長）、臨床所見を評価の上、今後、スポーツ復帰時期を決定する予定である。２症例とも現時点では創部感染や創の遷延治癒、腓腹神経刺激症状などの手術合併症はなく、痛みなく歩行できており、手術後の経過は良好である。今後は術後3,6,12ヶ月でMRIを行い、術後3,6,9,12,18,24ヶ月でアンケート、可動域、下腿周長、片足爪先立ち、臨床スコア(JSSFスコア、ATRS、SAFE-Q)を調査していく予定である。
