项目摘要

2022年度は、以下の2つの研究を中心に進めた。1つ目は、医療ソーシャルワーカー（以下、MSW）が保証人問題についての地域活動を行う上での阻害要因を質的に明らかにするために行ったインタビュー調査データ（MSW10人分）を分析し、学術雑誌（査読付き）に投稿し、採択・論文掲載された。内容面の成果としては、従来の先行研究では示されていない保証人問題と密接に関わっている阻害要因を確認することができた。さらに、地域活動実施の有無や所属地域、想定している地域活動のタイプの違いによって、阻害要因の内容や重みづけが異なっていることを明らかにすることができた。一方で、〔上層部の理解が得られにくい職場環境〕と「社会レベル」の要因をより重く捉えている可能性を示すことができた。本研究で得られた知見は、個々のMSWが地域活動を行ったり、職能団体がMSW対象の教育・研修プログラムを検討・開発する際の参考になるのではないかと考える。2つ目は、MSWが身寄りがない患者を支援する上での困難性や地域活動を行う上での阻害要因を量的に明らかにするために、全国の病院に所属しているMSW（3,000人）を対象にした質問紙調査を実施した。回答率は46.9％（1,407人）であった。回答データの入力作業と集計作業までは完了することができたが、2変量解析や多変量解析までは行うことができなかったため、1年の延長申請をした。2023年度中に、さらに分析を進めていき、学術雑誌への投稿をしたいと考えている。現段階で得られている内容面の成果は、以下の通りである。身寄りがない患者の支援といっても、判断能力の程度や成年後見制度を利用しているかどうかで、支援の困難性が異なっていることを実証することができた。さらに、大規模な量的調査を行ったことで、MSWが保証人問題についての地域活動を進める際の阻害要因の重みづけを明確に示すことができた。
