従来「心因性疼痛」と呼ばれていた痛みが近年の脳イメージング技術の発達により、「脳内で痛みを過剰かつ過敏に認知するようになってしまった病態」の痛み だと判明し、新たに提唱された「非器質的疼痛」の病態を解明するため、また「非器質的疼痛」に対する三環系抗うつ薬の脳構造・機能的な作用機序の解明のた め、典型的な「非器質的疼痛」疾患である非定型口腔顔面痛を対象とした研究である。非定型口腔顔面痛患者群の治療前後のMRI(functional MRIを含む)、心 理 検査等のアンケート結果と健常対照群のそれを比較する。患者群と対照群の比較により非定型口腔顔面痛の病態特異的な脳構造・機能ネットワークを抽出し、 患者群の治療前後の比較によりその変化を観察する。先行研究から、非定型口腔顔面痛に関して前頭前野における下行性抑制系の減弱が示唆されていること、 「非器質的疼痛」の類縁疾患である過敏性腸症候群患者に三環系抗うつ薬であるアミトリプチリンを投与し前帯状皮質の活動を抑制する結果が示されていること から、非定型口腔顔面痛の病態は前頭前野の異常興奮にあること、三環系抗うつ薬による治療を通して、その異常興奮が正常化する仮説を検証する。そして非定 型口腔顔面痛の病態、それに対する三環系抗うつ薬の作用機序を解明することで、「非機能的疼痛」全体の病態や治療機序を考察する端緒とすることを期待して いる。2020年度には患者群20例以上のMRIとアンケートのdataを集積した。 2021年度には健常群26例のMRIとアンケートを集積し一部解析を行った。2022年度には解析を完了した。
