イブルチニブはB細胞性造血器悪性腫瘍の高齢者に使用されるBruton’s tyrosine kinase（Btk）を不可逆的に阻害する経口分子標的薬である。高齢化が加速する日本において、イブルチニブによる治療を受ける患者が増加すると考えられる。イブルチニブによる細胞性免疫の低下が、重篤な転帰に至る肺アスペルギルス症などの侵襲性真菌感染症を誘発することが報告されている。しかしながら、イブルチニブ血中濃度と真菌感染症の関連を明らかにした報告はない。本研究の目的は、イブルチニブ使用症例における真菌感染症のリスク因子を探索し、イブルチニブ血中濃度と真菌感染症の関連を明らかにすることである。国内において、イブルチニブが適応となる慢性リンパ性白血病の症例が少ないため、医療ビックデータを用いてリスク因子を探索している。令和4年度では、医療ビックデータを用いて、イブルチニブの適応症である慢性リンパ性白血病症例の日本における真菌感染症の発症率が9.2%であることを明らかにした。また、慢性リンパ性白血病は病勢進行が緩やかなため、発症初期に抗悪性腫瘍薬を使用せずに経過を観察する。その期間における真菌感染症の発症率が1.5%であることも明らかにすることが出来た。さらに、イブルチニブが投与された慢性リンパ性白血病症例において、抗真菌薬の予防投与がイブルチニブによる真菌感染症の発症予防に寄与しないことを明らかにした。患者サンプルを用いたイブルチニブ血中濃度測定では、血液透析療法を受けている慢性リンパ性白血病症例のイブルチニブの薬物動態を明らかにした。

Bruton's tyrosine kinase (Btk) is an irreversible inhibitor of B-cell hematopoietic tumors. The treatment of patients with high blood pressure increased rapidly. The cellular immune system is depressed and re-infected, and invasive fungal infections are reported. The relationship between blood concentration and fungal infections is clearly reported. The aim of this study was to explore the risk factors for fungal infections in patients with fungal infections, and to clarify the relationship between serum concentrations of fungal infections and fungal infections. The number of cases of chronic leukemia in China has been reduced, and medical treatment has been used to explore the factors of chronic leukemia. The incidence of fungal infections in chronic leukemia cases in Japan was 9.2% in 2004. Chronic leukemia is a disease that progresses slowly and is detected early in life by anti-leukemia agents. The incidence of fungal infections during the period was 1.5%. In addition, anti-fungal drugs are used to prevent the development of fungal infections in patients with chronic leukemia. The measurement of blood concentration of Iberil in patients with can clarify the drug dynamics of Iberil in patients with chronic leukemia undergoing hemodialysis therapy.
