项目摘要

双極性障害は、躁状態、うつ状態などの病相を繰り返す、重篤で慢性かつ再発性の精神疾患である。躁病相あるいはうつ病相において、仕事や対人面といった社会機能の低下を認めることは以前より指摘されているが、気分症状のない症状寛解期においても、社会機能が十分に回復せず、いわゆる、「症状寛解と機能回復のずれ」が臨床場面で観察されることは少なくない。近年の知見の集積により、双極性障害では認知機能障害が、この「ずれ」と関連していることが明らかとなってきた。双極性障害の症状寛解期では、視覚学習、社会認知、処理速度、MATRICSコンセンサス認知機能評価バッテリー総合スコアにおいて、有意な機能低下を認めたと報告されるなど、客観的認知機能障害を呈することが知られている。さらに症状寛解期において、これらの客観的認知機能のみならず、主観的認知機能も障害されることが報告されている。このように、双極性障害では症状寛解期でも認知機能障害を呈するため、適切な維持療法および入念な心理社会的アプローチが必要であることが指摘されている。近年、症状寛解期における認知機能障害は、社会機能の障害との関連が注目されており、双極性障害の治療ターゲットとして認知機能障害を捉えることが重要と考えられるようになってきた。現在、認知機能障害は、双極性障害患者の機能回復と生活の質を向上させるための新しい治療標的となっている。本研究では、双極性障害症状寛解期における認知機能障害と事象関連電位、病識、社会機能との関連についての横断研究を行い、認知機能障害と関連する因子について、探索的検討を行った。
