项目摘要

大腸癌stageIVの治療成績は依然として悪く、遠隔転移の制御は喫緊の課題である。大腸癌の遠隔転移能は腫瘍が進行する過程で獲得されると考えられてきたが、近年、原発腫瘍の発生した初期の段階から、転移能をもったサブクローンは、全身に播種され、転移先臓器に浸潤しているということが明らかとなった。そこで我々は、根治切除後に異時性遠隔転移再発を来した早期癌症例を対象に遺伝子発現解析を行い、転移に関わる因子の同定を試みた。対象症例は異時性転移を伴う、もしくは5年無再発を確認したStageI-IIの早期大腸癌症例とし、転移症例については原発巣と転移巣の組織を用いた。コントロールには正常大腸組織を用いた。対象症例全例でMSI検査を行いMSSであることを確認した。cDNA microarrayによる網羅的発現解析の結果、正常大腸組織との比較における発現変動遺伝子は、異時性転移群で364遺伝子、無再発群で297遺伝子であり、エンリッチメント解析の結果、異時性転移群でのみ細胞接着や細胞外マトリックスに関する因子を上位に認めた。また、異時性転移群と無再発群の比較における発現変動遺伝子から、異時性遠隔転移再発に関わる因子として5遺伝子を抽出し、そのうち発現上昇を認めた3遺伝子を候補遺伝子として同定した。次に、StageI-IIIの切除検体のホルマリン固定パラフィン包埋組織のtissuemicroarrayを用いて免疫染色による発現評価を行い臨床病理学的因子との相関を検討したところ、いずれの候補遺伝子も異時性遠隔転移再発と有意に相関し、予後不良因子であることが判明した。また、候補2遺伝子について、タンパク質レベルでの解析を行い、発現の増加を認めた。現在は各候補遺伝子の遺伝子改変株を製作している段階で、今後、各分子の機能解析を予定している。
