次世代シーケンサーを用いたがん遺伝子パネル検査により、臓器別治療から遺伝子異常に基づく抗がん剤の選択が可能になった。一方検体採取部位の規定はなく、抗がん剤の奏効は採取された検体に依存する可能性がある。原発巣は壊死部や正常組織の混在、腫瘍増悪に関与する遺伝子異常を包含していない場合が懸念される。骨転移巣は、腫瘍細胞以外の混入が少なく、増悪の要因となる遺伝子異常を包含している可能性が高いと期待され、これに基づく抗がん剤の選択が奏効率の向上に貢献することを期待する。本研究の目標は、骨転移病巣と原発巣の遺伝子変異を、がん遺伝子パネル検査を通じて検討することである。2022年4月1日から2023年2月28日まで候補となる腎細胞がんの検体をホルマリン固定を行い核酸抽出を行いDNA検出したのが3件、RNA抽出が1件だった。核酸品質と核酸濃度確認を2セットに行ったが、いずれも品質不良のため解析に至らなかった。そのため2022年12月に解析対象の変更を検討した。原発と転移の両方揃う症例について多いのは肺がんだったが、原発巣が肺の場合には組織量が少ないため候補とするのが難しかった。当科で扱う骨軟部肉腫に関して、肺転移例の切除検体と原発治療が比較できる検体が5例以上あった。しかし肉腫は遺伝子変異が無い融合遺伝子の異常による発症が多く、かつ融合遺伝子は原発巣と転移巣で変化が生じていない可能性が高い。また用意している遺伝子パネルは融合遺伝子の検出には対応していないパネルだった。研究のコンセプトは原発、転移先での遺伝子の違いを確認し、転移先のほうが最新の遺伝子変化があり、転移先の遺伝子変異を調べることにより治療に反映できる可能性があることであった。その結果を出すことに研究の狙いがあるため、骨軟部肉腫を対象とした解析は断念し、腎細胞がんを対象としてこの度研究機関を1年延長申請させていただくことにした。
