分娩進行の評価法として手指による内診法が用いられているが、検者間・検者内誤差が大きいことが指摘されている。経会陰超音波検査(intrapartum-transperineal-ultrasound;以下ITU)は、恥骨と児頭の位置関係を客観的に評価でき、2018年にはISUOGからガイドラインが発表されたが、本邦では普及していないのが現状である。我々はITUによる客観的な分娩評価法の普及を目指し、陣痛促進剤開始時から分娩まで定期的に内診およびITUで診察した。パイロットスタディの期間も合わせ、2019年3月から2020年12月までの正期産、単胎の分娩誘発症例の、のべ約100分娩の蓄積をおこなった。ITU所見は主にangle of progression(AOP)を使用した。1)内診とAOPの関連を調べた。2)時間軸で描いたAOP曲線を作成し、初産群と経産群および自然分娩群と器械分娩群で比較した。子宮口8cm〜全開大後に、急速に児頭の下降が見られるため、より正確なAOPによるパルトグラムを作成するために同時期からは20-30分おきに診察をすることに変更した。これらデータの蓄積により、正確な分娩進行の把握、回旋異常や遷延分娩などの診断が可能となった。分娩の高い安全性を保つためには、より客観的な分娩評価方法の確立が必要であり、本研究結果は極めて有用である。

The method of evaluation of labor progress and the method of internal diagnosis are used in the middle of the examination, and the error in the examination is large. Intra perineal-ultrasound (ITU) is a new type of ultrasound examination. In 2018, the ISUOG will be launched and the popularity of the ultrasound examination will be improved. We are committed to the ITU's goal of popularizing the birth assessment method, promoting regular visits at the beginning of labor, and ITU's examination. From March 2019 to December 2020, there are about 100 cases of labor induction, labor induction and labor accumulation. ITU found that the main angle of progression(AOP) was used. 1)Internal diagnosis and AOP 2)The Timeline describes the creation of AOP curves and the comparison between the first birth group and the second birth group and the third birth group. The uterus is 8cm wide, and the head is rapidly descending. The diagnosis of delayed delivery is possible. The results of this study are extremely useful and necessary for the establishment of delivery evaluation methods to ensure high safety of delivery.
