脳腫瘍術前の高齢者における咬合力の測定を行った。術前口腔内環境調査ではう歯、欠損歯などの既存疾患が多い傾向にあった。動揺歯や治療が必要な歯もあり術後の咬合力、口腔内環境に影響を及ぼす可能性が示唆された。前年度と比べ観察期間、症例数が増えたため次年度の経過にあわせて統計学的な解析を予定する。術前後で咬合力を測定できた症例については、術後、１週間後に特に咬合力が明らかに低下する傾向がみられた。術後疼痛や側頭筋切開の影響が示唆された。開口制限がみられる症例が昨年同様みられた。側頭筋切離との関連が想定されたが経過観察期間が不十分であり今後の継続的な観察が必要である。術前ガムによる訓練は完全な訓練ができず不十分となった症例が多かった。その原因としては高齢者を対象とした研究であるが実際にエントリーする症例が高齢であり、ガムを噛むことができない症例が多かった。エントリーの段階でガムを噛めるかの確認を行っているが実際かみ始めると義歯が取れそうになる等の理由で途中リタイアする症例が多かった。結果として術前咬合力訓練が実施できた症例が1例と非常に少ない結果に留まった。研究が継続できている対象者の中で最も長い期間観察できている対象者は12ヶ月の経過を観察している。血液学的所見としては側頭筋切離による手術手技は採血データに影響しない傾向がみられた。開口制限に関しては術後６ヶ月ほどで改善する傾向がみられたが術前に比べ悪化している症例が多くみられた。側頭筋の萎縮に関連すると想定される４つの因子を術中に観察したが、現状の結果では側頭筋の切開法に関与がないことが判明した。コロナ禍の影響で目標症例数には到達していないが2020-21年度に比べ登録症例数が増えており引き続きの登録、観察、解析が必要である。
