人工呼吸器や経管栄養など、日常的に高度な医療を必要とする「医療的ケア児」を対象とした小児在宅歯科医療は、ニーズが増加しているものの、まだ広く普及していない。小児在宅歯科医療のエビデンス構築と在宅での口腔管理システムの開発を目的とした調査研究を継続している。今年度は、18トリソミー児12名（男児3名、女児9名）を対象として、在宅歯科医療の実態について調査を行った。在宅移行時の平均年齢は7.2か月であり、歯科初診時の平均年齢は18.7か月であった。医療的ケアは、人工呼吸器11名、在宅酸素12名、気管切開3名、吸引11名、経管栄養10名であった。初診時の嚥下機能は、唾液嚥下困難２名、唾液嚥下ほぼ可能8名、全量経口摂取2名であった。主訴は、口腔ケアに関する内容7名、摂食嚥下に関する内容5名であった。介入期間は、1年未満4名、1年以上２年未満４名、２年以上３年未満２名、３年以上２名であり、介入内容は、口腔衛生管理12名、摂食機能療法9名、歯科処置（歯石除去、シーラント、抜歯等）5名であった。転帰は、継続8名、死亡4名であった。歯科訪問診療は、在宅移行後一定の期間を経て開始されており、在宅生活が落ち着き、歯の萌出等をきっかけに歯科につながった児が多いと推察された。今回調査した18トリソミー児のうち30％が死亡転帰であった。在宅移行後の18トリソミーの生命予後の予測は困難であり、歯科的介入の選択は医科主治医等と連携しながら慎重に判断する必要があると考えられた。

During the intervention period, there were 4 in 1 year, 4 in more than 2 years, 2 in more than 3 years, 2 in more than 3 years, 12 in intervention content, 12 in oral health management, 9 in food machine energy management, and 5 in clinical practice. There were 8 deaths, 8 deaths and 4 deaths. After the transfer of the house, they must start to have a holiday, stay in the house, sprout, and so on. This time, I was told that I would die at least 30% of my life. After the transfer of the house, the rest of your life will be rewarded for the rest of your life, and the intervention of the department should be selected, such as the attending physician of the medical department, and so on, so as to make a careful judgment.
