昨年度は、東海地方４県（愛知県、三重県、岐阜県、静岡県）の訪問入浴介護事業所184か所に従事する看護師368名を対象に、看護師が対象者の入浴可否判断に用いる判断指標とその判断基準、訪問入浴介護に従事していて入浴可否判断に迷った事例や急変した事例について無記名の自記式質問紙調査を行った。今年度はその内容の分析を行い、次の研究の課題を見出した。特に、質問紙調査の結果として、看護が判断に迷う事例から生成された内容は、「普段より循環動態が不安定である」「普段より体温が高いあるいは低い」「普段より意識レベルが悪化している」「ターミナル期の利用者で全身状態が不安定である」「普段より呼吸状態が悪化している」など8個のサブカテゴリーから「普段より利用者の全身状態が不安定で訪問入浴中・後の利用者の安全が保てない可能性がある」のカテゴリーを、「利用者の全身状態が安定していないが利用者本人・家族の希望がある」「利用者本人や家族が入浴を拒否される」のサブカテゴリーから「利用者本人、家族、医療者の入浴への意向が異なる」のカテゴリーを生成した。訪問入浴に従事する看護師は、利用者の全身状態の不安定さから入浴の負荷に耐えられる状態か判断に迷うことが多いと考えられた。また訪問入浴の判断は利用者や家族の意向、医師や訪問看護師の判断も影響され、複雑な入浴可否の判断を求められることが示唆された。それらの結果から、教育プログラムを作成していく際には、訪問入浴介護事業所内だけの連携ではなく、家族や主治医、訪問看護師との連携も加えていく必要があることが示唆された。
