项目摘要

2022年度は、重度の精神症状を呈する統合失調症患者の訪問支援に関する実践的教育プログラム（以下、教育プログラム）の作成と実施予定として準備を進めた。まず、2021年度までの文献検討、精神医学・保健・看護系の学会での情報収集をもとに、教育プログラムの対象者、対象施設、内容の基盤となる理論の選定や教育プログラムの構成について検討を行った。対象施設は、精神科訪問看護を実施している訪問看護事業所とした。プログラムを受講する看護師については、重度の精神症状を呈する利用者への困難感は、経験年数によらずすべての看護師が抱くと先行研究より推察されたため、本教育プログラムを受講する対象看護師の看護師経験年数および精神科訪問看護経験年数は問わないこととした。さらに、教育プログラムの基盤となるモデルとして、困難な看護実践を振り返り新たな実践へとつなげるために活用されていると先行研究で報告されているリフレクションのプロセスを採用することとした。リフレクションに関する文献を調査し、プログラムの構成を検討する中で、日中は訪問でほとんど事業所に戻らないことも多い訪問看護師の働き方を踏まえ、訪問看護師へ実施可能な教育プログラムにするための形式や方法について検討を行った。検討を行ったプログラム案を、精神保健・看護の実践者と研究者が集まっている、研究者が所属する研究機関の精神看護学研究室ゼミで発表し、意見交換を行った。教育プログラムの対象や内容、実施形態、内容について意見をもらい、教育プログラムをさらに洗練させた。洗練させた教育プログラムについて、次の段階として実施のための準備を進める必要があったが、新型コロナウイルスの流行の波が断続的に発生し、訪問看護ステーションの業務負担が多くなる時期が続いた。そのため、感染状況や、訪問看護ステーションの業務状況を注視しながら、研究再開が可能な時期を検討していた。
