项目摘要

地域で生活する統合失調症者のSOC（Sense of Coherence：首尾一貫感覚）に関する文献検討を行った。特定集団の当事者である自身の自身に対する否定的な考え方であるセルフスティグマは、当事者のパーソナルリカバリーを妨げるものであるが、SOCがセルフスティグマの影響を緩和することが報告されている。各概念は関連しあっていることが推測され、今後、SOC向上のための介入プログラムの検討に向けて、SOC、セルフスティグマ、パーソナルリカバリーに関する調査を行った。背景要因をある程度均一化するため、「中四国地方の精神科デイケアを半年以上利用しながら地域で生活している18歳以上の統合失調症者」を対象とした。COVID-19の影響が継続していること、当事者に回答を依頼するにあたりオンラインでは困難が想定されることから、郵送での調査とした。2022年9月から2023年2月にかけて、15施設計270名に調査票を送り、149名より返送があった。そのうち、研究協力に同意の得られなかったものや対象者の選択基準に合致しないものを除いて140名の回答を分析対象とした。セルフスティグマがパーソナルリカバリーに影響を与える中で、SOCがセルフスティグマ、パーソナルリカバリーそれぞれに影響を与えるという仮説モデルを設け、構造方程式モデリングにより概念間の関連を検討し、その適合度は概ね妥当であった。SOCは直接パーソナルリカバリーを促すのではなく、セルフスティグマの影響を軽減することでパーソナルリカバリーを促進すると考えられた。活動を通して当事者が自身の経験を振り返る機会をもち、グループ間でその経験を共有できるようにするとともに、支援者自身の支援を振り返ることで、統合失調症者のセルフスティグマの軽減およびSOCの向上へとつながり、ひいてはパーソナルリカバリーが促進される可能性が示唆された。
