令和2年4月から生体肝移植目的（LDLT）で長崎大学病院消化器内科入院となり、LDLTが行われた非代償性肝硬変患者において、6例の患者から同意を頂き、3例の患者にLDLT後の電気刺激療法（NMES）および分岐鎖アミノ酸製剤（BCAA）併用投与、3例の患者にNMES単独療法が行われた。筋肉量に関しては、体組成分析装置であるInBodyおよび超音波診断装置における大腿四頭筋の厚さを測定し評価した。筋力に関してはスメドレー式握力計による握力を評価項目とした。全例、LDLT1ヶ月後の筋肉量および筋力の著明な低下が見られたが、LDLT3ヶ月後に関しては症例毎に筋肉量回復の程度に差が見られた。今後も、50症例を目標に臨床研究を継続するが、新型コロナウイルス感染症により肝移植症例が例年より少なかったため、本年度は少数例に留まったと考えられる。今回、留学のために研究中断となったが、2023年7月以降に上記臨床試験を再開する予定である。また、不動化したラットにNMES＋BCAA併用を行い、筋肉量や筋組成の変化について検討する動物実験に関しては、コントロール群、不動群、NMES群、BCAA群、NMES+BCAA群の5グループ、各群ｎ＝６を設定し、14日間に渡り各処置を行った。各群における筋肉量、筋核数等に関しての解析は完了していないが、研究再開とともに、各群間における比較検討を行う予定である。また、筋肉量以外に、各群における関節可動域も測定しており、NMES＋BCAA治療における拘縮予防効果についても解析予定である。
