项目摘要

高次脳機能障害者への科学的根拠に基づいた就労支援プログラムは開発されていない．高次脳機能障害者は症状が多様であり，体系的な支援が難しいとされている．障害種別の就労率について先行研究を見ても，発達障害や知的障害は60％を超えているが，高次脳機能障害は約45％と他の障害に比し低い現状にある．高次脳機能障害者に対する認知行動療法やパーソナルコンピューターを活用した認知機能訓練，ピアサポートグループ，ワークサンプルの活用に関する効果は示されているが，それらの成果が就労につながっているかは未解決の課題である．本研究では，高次脳機能障害者に対する科学的根拠に基づいた就労支援プログラムを開発したい．10名の高次脳機能障害者で就労（復職は除く）を目指している方にパーソナルコンピューターを活用した認知機能訓練とワークサンプルを活用した職業訓練を併用したプログラムを提供した．対象者の基本属性は，男性9名，女性1名，年齢は平均41.7±8.1歳，診断名は脳梗塞4名，脳出血1名，頭部外傷2名，くも膜下出血1名，びまん性軸索損傷1名，低酸素脳症1名である．認知機能訓練は高次脳機能バランサー（製品：レデックス株式会社），ワークサンプルはワークサンプル幕張版（製品：障害者職業総合センター）を活用した．アウトカム指標は，神経心理学的検査と就労（再就職及び就労継続支援A型）の可否とした．訓練時間は，個別で週1回40分実施し，課題難易度と課題内容は対象者の能力に合わせて設定した．
