変形性股関節症の一要因と危惧されている股関節不安定性の実態は不明瞭な点が多い。本研究では、レントゲン画像などで描出できない股関節の微小な不安定性を評価するmicroinstability test時の生体内での大腿骨頭の動態を明らかにし、偏位量を定量化し、検査の妥当性や病態との関連性を調査する。2023年3月時点で股関節不安定性を惹起する可能性のある46症例（寛骨臼関節唇損傷10例、寛骨臼形成不全35例、股関節包損傷1例）のデータを取得済である。2022年度は本研究に関連した成果を、学会にて一般演題3題（北海道整形災害外科学会、日本整形外科超音波学会、日本股関節学会）、シンポジウム1題（臨床スポーツ医学会学術集会）発表した。そのうち日本股関節学会では、「優秀演題アワードセッション（リハ部門）」にエントリーされ、股関節領域での本研究テーマの関心の高さが認識できた。また、シンポジウムではアスリートの股関節障害に対して、昨年度公表された論文データを基に股関節不安定性の関連を述べ、股関節不安定性の実態について本実験データを紹介してその臨床的意義を解説した。これらの発表では、寛骨臼形成不全患者、寛骨臼関節唇損傷患者、股関節包損傷患者におけるtraction test中の大腿骨頭変位量とその関連因子を検討した。結果として、テスト中の自覚的な亜脱臼感の有無で変位量に差は認めなかった。また、関節唇損傷股では有意な変位量を認めなかったが、寛骨臼形成不全股における亜脱臼群、関節包損傷患者では大きな変位量を認めた。以上より、大腿骨頭変位増大に関連する因子は寛骨臼形成不全に伴う亜脱臼、そして関節包損傷である可能性が示された。次年度は、本年度の成果の論文化に努めたい。

One of the main causes of variant femoral arthrosis is crisis. The status of femoral joint instability is unclear. The purpose of this study was to investigate the relationship between the dynamics of thigh bone in vivo, the quantification of deviation, the appropriateness of detection and the pathology during microstability test, and to evaluate the microinstability of femoral joints. In March 2023, 46 cases (10 cases of hip joint labrum injury, 35 cases of hip joint incomplete formation, 1 case of hip joint capsule injury) of femoral joint instability were diagnosed. In 2022, the results of this study were presented in three general questions (Hokkaido Society of Plastic and Disaster Surgery, Japan Society of Plastic Surgery and Ultrasound, Japan Society of Femoral Joint) and one general question (Clinical Society of Medical Sciences). The Japanese Joint Society of Science and Technology (JSC) is committed to the development of joint science and technology in Japan. The present paper discusses the relationship between femoral joint instability and the status of femoral joint instability, and explains its clinical significance. The correlation factors of thigh bone displacement in traction test were analyzed in patients with incomplete hip joint formation, hip joint lip injury and hip joint capsule injury. The results show that the difference between the dislocation feeling and the position difference is obvious. For example, if the joint lip is injured, the patient will have a large amount of dislocation due to the formation of a wide bone socket. The above factors are associated with incomplete bone dislocation and joint capsule injury. Next year, the results of this year's discussion of cultural efforts.
