项目摘要

2022年度はグルテン不耐症の症状や抗体価についての検討を行った。兵庫医科大学病院グルテン専門外来を受診した20歳-70歳のグルテン不耐症と、対照群としてグルテン感受性に関連する症状の自覚のない20歳-70歳を対象とした。血液を採取し、ELISA法にて抗グリアジンIgG抗体などのグルテン関連抗体を測定した。また精神症状、身体症状を問診および自己記入式質問紙について評価した。グルテン不耐症群では、消化器症状、易疲労感、睡眠関連問題を有する割合が高く、身体症状だけではなく、うつや不安の症状を有する割合も高かった。免疫学的グルテン感受性群では、非免疫学的グルテン感受性群に比べ、自覚症状を有する者が多く、過敏性腸症候群の診断基準を満たす者やQOLの低い者が多かったことから、消化器に影響を及ぼしていると考えられる。今回、抗グリアジンIgG抗体陽性の感度は38%と低く、バイオマーカーとして抗グリアジンIgG抗体のみを用いても、グルテン不耐症の診断には十分ではないと考えられた。グルテン不耐症の正確な診断基準を確立するためには、複数のバイオマーカーを組み合わせて使用することが望ましく、抗グリアジンIgG抗体だけでなく、他の客観的な指標も必要であると考えられた。これらの結果は第118回日本精神神経学会学術総会、22nd IUNS-International Congress of Nutritionで報告を行い、論文化を予定している。現在、うつとグルテン不耐症の関連についての解析を行っている。
