小児思春期の過敏性腸症候群における低FODMAP食とグルテン不耐症に関する研究

项目摘要

令和4年度は、反復性腹痛と便通異常（便秘・下痢）を主訴とする小児・思春期の患者およそ100例を対象として、器質的疾患（炎症性腸疾患、好酸球性胃腸炎など）の鑑別のため内視鏡検査を行った。このうちRome IV分類に基づきおよそ30例を過敏性腸症候群（IBS）と診断した。ほぼ全例に低FODMAP食を提案し、このうち若干名が小麦の制限で症状の著明な改善を認めた。小麦除去で症状が改善する症例では、I型アレルギーとしての小麦アレルギーあるいは好酸球性胃腸炎が否定されていることから、どのような機序で小麦が症状の発現に関与していたのか、今後さらに症例を増やして検討する必要があると考えられた。一方、我々の先行研究において、炎症性腸疾患あるいは好酸球性胃腸炎と診断した患者の中にも、小麦除去で腹痛や便通異常の改善する症例が若干名いることが明らかになった。このためまずは炎症性腸疾患を有する症例において、セリアック病のスクリーニング検査である血清抗組織トランスグルタミナーゼIgA抗体（tTG-IgA）を測定したところ、31例中1例がtTG-IgA抗体が陽性であり、小麦除去で症状の改善が確認された。当該症例では、好酸球性胃腸炎や抗原特異的IgEが陰性であることから、日本ではきわめて希な疾患と考えられてきたセリアック病が潜在している可能性が示唆された。以上の結果から、小児・思春期の過敏性腸症候群において、小麦除去が有効な症例の臨床的特徴を明らかにし、新たなバイオマーカーの検討ならびに治療につながる病態の解明を推進することに大きな意義があると考えられた。
