2021年度は、研究の４つの柱のうち心不全患者の再入院要因研究、病いの経験研究に重点を置いて進めた。以下、４つの柱別に説明する。①心不全患者の再入院要因研究:心不全患者の前向き観察研究により、再入院の社会的・医学的要因を明らかにすることが目的である。協力病院との新型コロナ感染症対策を含めた調査方法の協議、心不全再入院要因に関する先行研究のレビューなどをし、研究倫理審査に必要な文書を作成した。統括者の所属大学、協力病院の研究倫理審査委員会において承認された。②心不全患者の病いの経験研究：研究の目的は心不全者の生活や仕事上の困難とその対処戦略、病いをもつ自己の意味の受け止めの変遷を明らかにすることである。実績としては、a.協力医療機関の研究倫理審査委員会で承認を得た。b.協力医療機関の医師、医療ソーシャルワーカーと調査対象となる心不全患者像の確認を行った。c.調査対象である心不全患者数名に調査概要の説明を行い協力についての同意を得た。③社会的処方研究：日本社会においても新たな課題となっている社会的孤立について先進的な取り組みを行っているイギリスの対策について、文献やインターネットから情報収集を行った。孤立・孤独である人々と地域資源とをマッチングする社会的処方のリンクワーカーの所属機関や役割を検討した。④地域連動型支援プログラム開発研究：大島巌らの効果モデルの構築に関する理論、および、源由理子の参加型評価に関する理論を基礎にしている。これらをもとに、プログラム開発の進め方について科研の研究者での検討会、および現場の実践者を交えての検討会を行った。効果モデル構築のステップ作業を確認したうえで第1次効果モデルの試行版の完成を目指すことになった。また、参加型評価については、実践者がどの程度関わるかは継続的な課題と確認した。
