项目摘要

本研究では、我が国の文化に即して、多様な価値観を持つ家族を理解し、「その人にとっての最善」を家族と共に考えることが重要であることに注目し、『家族との協働型エンドオブライフケア実践能力を高める看護遠隔教育プログラム』を開発し、効果を検証することを目的とする。本年度の目標は、第１局面である家族との協働型エンドオブライフケアの看護支援行動、コンピテンシーの構成要素の特定を目的とし〈Step1～３〉に取り組んだ。Web会議システムを用いて全体会議を４回開催した。〈Step1〉では、研究者らが開発したケアガイドラインも参考にしながら国内外の過去10年程度の文献を収集し、家族との協働型エンドオブライフケアの看護支援行動（案）を抽出し、現在、文化的特徴を考慮し検討している。〈Step２〉では、日本国内の既存の文献や学会から発表されているエンドオブライフケアの看護実践能力における書籍やガイドライン、ファミリーセンタードケアガイドライン等の海外文献等を用いて、システマティックレビューによる家族との協働型エンドオブラウフケアのコンピテンシーの抽出に取り組んでいる段階である。これらと並行して〈Step３〉の研究者間でブレインストーミングを行っている。また、本研究プログラムを看護学士教育卒業時の「理解できる」レベルから、病院、在宅、施設など、あらゆる場でその人と家族のエンドオブライフを「支えることができる」レベルへと看護実践能力を高めていく効果的な継続教育プログラムと位置付け、日本看護協会のラダーレベルⅢの看護師が理解できる内容とし、本遠隔研修プログラムを終えて、学んだ知識を活用して家族と協働してエンドオブライフケアができることをゴールとした。家族と協働するためのエンドオブライフケアに必要な基礎知識とする【総論】と【各論】の２部構成とする教育プログラムの骨子の作成を現在検討している。
