継続して尿検体採取をすすめ、膵癌症例73例、他の膵腫瘍症例13例、健常者6例の総計92例分の回収を終了した。これらに関してまずcell free DNA（cfDNA）を採取した。採取濃度は疾患に依存する結果となったが、cfDNAの品質に関してはさらなる解析を行うに足るacceptableな結果であった。各cfDNA検体について、bisulfite処理を済ませ、当科保有のメチル化マーカーパネルから3つのメチル化マーカーを選択し、定量メチル化アッセイ（QMSP）を施行した。これらの症例の臨床データとして、年齢・性別に加え、疾患名、担癌状態か、化学療法施行後か、切除術施行後か、血清腫瘍マーカー、血清腎機能マーカーを電子カルテから抽出し、結果解析に用いた。膵癌症例の尿検体から抽出したcfDNAからは、他の症例からのものよりも有意に多いメチル化シグナルを同定でき、これらの尿中には癌由来核酸がしっかり含まれていることが強く想定された。一方で膵癌症例の中で3つのメチル化マーカーの陽性数はそれぞれ18例、5例、4例と想定を下回る陽性率であり、cfDNA採取過程、あるいはbisulfite処理過程でのDNA量の減衰があるものと考えられた。しかし現状ではこれ以上感度向上につながる手段を持ち合わせないため、引き続き同様のアッセイを用いることとした。Tumor heterogeneityに配慮して１つでもメチル化マーカー陽性の症例をQMSP陽性と考え、膵癌73例をQMSP陽性20例と陰性53例に分けて臨床データと比較した。腫瘍切除施行済み症例ではQMSP陽性率が少ない傾向にあった（P=0.053）ほかは、有意に相関する因子は認めなかった。血清腫瘍マーカー値も両群間に大きな差は認めなかった。また遠隔転移を有する症例で陽性例が多いことを想定していたが、明らかな傾向は認めなかった。

The positive rate of QMSP was significantly lower than that of the control group (P=0.053). Seroma is a serious disease. There are many positive cases in which the distance between the two sides is different.
