项目摘要

過度な偏咀嚼の持続は、歯の破折や咬耗、顎機能障害の要因の一つと考えられている。私たちは、これまで偏咀嚼に着目し、片側臼歯部欠損患者が健常者よりも偏咀嚼を示すこと、また片側臼歯部欠損患者に対する補綴治療(インプラント固定性補綴装置・可徹性部分床義歯)が偏咀嚼を改善することを明らかにした。しかしながら、遊離端欠損に対して必ずしも第2大臼歯まで補綴治療せず対応する短縮歯列(Shortened Dental Arch)は臨床的に問題ないという概念がある。一方本邦では、欠損部位はすべて補綴治療する考えが一般的であり、第2大臼歯までの補綴治療が広く行われている。しかし、第2大臼歯まで必要かどうかという根拠は不足している。そこで、本研究は片側臼歯部遊離端欠損患者の偏咀嚼に着目し、クロスオーバー比較試験を用いることで、補綴設計(第1大臼歯までの補綴、第2大臼歯までの補綴)が偏咀嚼に与える影響を明らかにすることを目的とした。現在、インプラント固定性補綴装置・可徹性部分床義歯治療を行った片側臼歯部欠損45名に加え、片側臼歯部欠損が存在するにも関わらず、欠損補綴を行わっていないものの偏咀嚼データを収集している。また第1大臼歯まで・第2大臼歯までの補綴設計でのクロスオーバー比較試験の準備を進めている段階である。
