项目摘要

客観的な診査所見には特に問題はないにもかかわらず、慢性的な咬合違和感を訴え、その改善を執拗に求める患者に遭遇することがある。咬合違和感の原因として精神疾患あるいは抹消から中枢神経系における情報伝達・情報処理機構があげられるが、診断方法が確立していない。したがって、咬合違和感を訴える患者に対する診断方法の確立は咬合違和感を訴える患者に対する補綴学診断および治療方法の確立に向けて有用であると考える。本研究では、咬合違和感患者の原因を分類することを目的とし、定量的感覚検査(Quantitative Sensory Testing：QST)を用いて咬合違和感を訴える患者の違和感のある部位の歯肉および患側の咬筋の感覚機能を健常者と比較する。また、精神健康調査(General Health Questionnaire：GHQ)を用いた心理社会的因子を咬合違和感を有する患者と健常者で比較し客観的な定量化を目指す。令和元年度に日本大学松戸歯学部倫理審査委員会の審査を受け、研究実施の許可を得てデータ収集を開始した。令和４年度は、８名の健常者および２名咬合違和感を訴える患者の定量的感覚検査（Quantitative Sensory Testing：QST）と精神健康調査（General Health Questionnaire：GHQ）を用いた検査を行い、データ解析を進めている。
