本研究の最大の目的は、家庭血圧計を用いて測定した夜間血圧の変動性の増大が、高血圧性臓器障害の進展や心血管イベントと関連する、という仮説を検証することである。一昨年度、解析対象とするデータベースの固定を行い、我々の研究グループは、夜間収縮期血圧平均値の10 mmHgの上昇が、診察室血圧や早朝家庭血圧レベルで補正後も心血管イベントリスクを約20％上昇させることを過去に報告した(Hypertension. 2019;73:1240-1248)。そして、昨年度は、同データベースを用いて解析を進め、家庭血圧計で測定した早朝血圧と就寝前血圧レベルの平均値は135/85 mmHg未満にコントロールされているが、同一血圧計で測定した夜間血圧レベルが120/70 mmHg以上とコントロールされていない集団を、「仮面夜間高血圧」と新しく定義し、その集団の心血管イベントリスクを検討した。観察期間中央値7.6年(19519人年)で162例の心血管イベントを確認したが、仮面夜間高血圧患者の心血管イベント発症に対するハザード比は1.57であり、有意な心血管イベントリスク因子であることがわかった。つまり、これまで我が国での高血圧治療ガイドラインで推奨されていた早朝起床後と就寝前の家庭血圧測定では、心血管イベント高リスク患者を見逃す可能性があり、夜間家庭血圧測定の重要性を明らかに示すことができた(Hypertension. 2020;76:259-266)。本年度は、夜間血圧変動性の指標と、心血管イベント発症との関連に関する解析を進めたが、現時点では有意な結果を見いだせていない。血圧変動性の指標や発症したイベント内容を含め、今後さらに解析を進めていく方針である。
