项目摘要

本研究の目的は、ロボット支援手術を受ける前立腺がん患者のサバイバーシップ支援充実に向けたエビデンスを明らかにすることである。具体的内容は、ロボット支援手術を受けた前立腺がん患者を対象に、手術前から術後1年間のQOL、自己効力感、日常生活体験について評価を行い、効果的な介入時期や内容、介入対象者についてコンセンサスを得る。そして、問題解決療法およびセルフモニタリングをベースとした支援プログラムを作成し介入研究を行い、その有効性の検証を行うことである。2022年度は、新型コロナウイルス拡大に伴い、作業に遅れが生じた。介入研究の実施が難し状況であったため、ロボット支援手術後患者のQOLに関する更なる分析および成果発表を行った。1）術後患者のQOLと失禁およびオムツ・パッド関連について分析を行った。術後尿失禁に対する長期的なニーズが示された一方で、包括的QOLと失禁回数・パッド枚数に有意な関連が示されなかった。2）術後患者のQOLと治療満足度について分析を行った。術後の治療満足度は高い傾向にあり、経時的変化を示さなかった。単変量解析の結果では失禁と治療満足度に有意な関連が示されたが、多変量解析では有意な関連を示さなかった。これからの結果から、術後患者への支援として、術後尿失禁ケアに加え生活全体を支援することがQOLや治療満足度の向上に重要だと示唆された。現在、研究成果について論文投稿作業を進めている。
