项目摘要

2019年度の目的は、内科的治療を行う急性大動脈解離患者（以下、患者）に対する看護実践力向上を目指した教育プログラム案の構築するため、集中ケア認定看護師（Intensive Care Certified Nurse；以下ICCN）と急性重症患者看護専門看護師（Critical Certified Nurse Specialist；以下 CCNS）が行う看護実践の内容と判断の根拠を明らかにすることであった。研究者の所属する施設の倫理審査委員会の承認を経てスタートした。研究方法は、看護師に対して内科的治療を行う患者に対する看護実践（特に発症直後の患者に対する看護）を想起してもらい、１）患者へ看護援助を行う上での着目点、重要と考えていること、２）看護師が意図的に患者への関わり持った場面や特徴的な援助を行った場面とその時の思考過程、３）内科的治療を受ける患者が快適に療養するために必要な環境についてそれぞれ①患者の集中治療室入室時②集中治療室入室後～入室3日目③集中治療室4日目～退室までに分け、半構造化面接にて聴取した。対象は高度実践看護を行う、ICCN及びCCNSとし、実践内容とその根拠、判断の様相についてデータ収集を行った。対象は主に、北陸三県において登録されているICCN,CCNSとした。加えて研究協力の同意が得られた研究参加者からの紹介によるスノーボール方式にて募った。ICCN5名、CCNS3名の計8名のインタビューを行い逐語録とし、グラウンデッドセオリーアプローチを参考に分析を進めている。
