【研究目的】ICUで治療を受ける重症患者の生存率は上昇している一方、ICUを退室した患者のQOLは低下していることが問題視されている。原因として、ICU退室後も持続する身体機能の低下、認知機能障害、PTSDやうつ症状といったメンタルヘルスの問題を持つことが挙げられている。海外では重症患者のみならず、その家族もメンタルヘルスに問題を抱えていることが明らかになっている。ICU退室後患者と家族のフォローアップのあり方や体制を構築していく必要がある。しかし、本邦においてICU退室後の患者とその家族のメンタルヘルスに関する追跡調査は為されていない。本研究はICU退室後患者とその家族のメンタルヘルスの実態と、現在のQOLとの関連を明らかにすることを目的とする。【研究実施計画】本研究は《課題1》アセスメントツールの選定と前向き調査の準備《課題2》メンタルヘルスとQOLに関する前向き調査の実施を計画した。《課題2》はICUに入室した患者とその家族を対象に、ICU退室後のメンタルヘルスとQOLについて追跡調査を計画した。しかし、新型コロナウイルス感染症により、家族の面会制限が生じた。ICUに入室した患者の家族を対象とした調査が困難となったため、ICU退室後の患者の追跡調査がなされていない本邦の現状を踏まえ、ICU退室後患者における長期的メンタルヘルスの実態を調査し、関連因子に家族の影響を加えて検討をする計画へ変更とした。【研究実績】《課題1》ツールの選定と前向き調査の準備は終了。《課題2》ICU退室後患者の長期的メンタルヘルスの実態と、関連因子に家族の影響が含まれるソーシャルサポートを加え研究計画を変更し、研究実施施設の倫理審査委員会より承認を受け、2022年に調査終了。調査結果を学会発表し、論文投稿中である。
