当該年度は、研究計画に則り、相談支援専門員を対象に、「医療的ケア児の在宅療養維持期における相談支援専門員の訪問看護師との連携課題及びその対処」について半構造インタビュー調査を行った。A県内において、計画相談支援および障害児相談支援を実施している相談支援事業所全箇所に、段階的に調査依頼を郵送し、研究参加者を募った。感染症流行時期ということもあり、オンラインインタビュー対応も可能としたところ、7名の相談支援専門員にインタビューすることができた。相談支援専門員は、担当となった医療的ケア児の医療面に関することや、相談支援と訪問看護のサービス提供体制や保険制度上の報酬体系の違い、訪問看護師との接点のとりにくさ等の課題を挙げていた。また、医療的ケア児とその家族の生活全体を見据え、児と養育者の意向と、児の安全面や養育者の介護負担を考慮した関わりにおいては、他の家族成員の生活も考慮したケアの提案が必要であることも述べられた。医療的ケア児と養育者のみを中心として捉えてしまい、他の家族成員の成長にも合わせなければならない現状と課題も垣間見ることができた。課題に対する対処として、同僚や訪問看護師に相談することで知識の補完を行うことや、関係職種へのアプローチの方法の工夫などを行い、円滑な連携を図ろうとしていた。また、今までの経験の蓄積とネットワークを活用しながら対応を模索していくことも語られた。この結果をもとに、再度先行研究や各種統計資料を確認し、訪問看護師へのインタビュー内容を再検討した上で本研究のデータ収集を継続していく。
