项目摘要

児童や高齢者は家屋内の生活時間が長く、乱雑な住居での生活はアレルギー関連疾患の他、転倒や火災など健康への影響も懸念される。筆者は、整理整頓が苦手な12歳～54歳を対象とした家庭訪問を含む整理整頓教育が一定の効果がある事を見出したが（ASO,et al.2021）、本研究においては多世代汎用型・ユニバーサルデザインの整理整頓教育プログラムを開発し、対象者の住環境を改善する事により健康状態の向上を目指す事を目的とした。まず、「ため込み症予防に向けた地域看護職の役割」を文献レビューし、地域看護職は対象者理解と関係機関との連携を重視しながら、支援システムを作り、解決をはかっている事、そしてその方法において「ため込み行動そのものを中止させるのではなく、安全面・健康面に注力し、対象者との信頼構築を図りながら、在宅生活の維持継続を図ること」を主流としていた事を見出した(ASO,2021)。これらの知見は主に、ため込み症対応先進諸国からの報告であった。そこで、2022年度は、我が国における生活衛生上課題のある在宅療養者を家庭訪問により支援する、様々な専門職の支援実際について調査を行った。対象とした職種は保健師、理学療法士、コミュニティソーシャルワーカー、環境衛生監視員であった。調査の結果、アセスメントの視点は、職種ごとに異なる一方、支援の方向性は「在宅生活が継続できるように」「ご本人の価値観を尊重した」等、一致していた。また、具体的な改善へ結びつける方策として、「片づけそのものをリハビリの一部とする」「片づけや観察を通して、近隣や関係機関との接点を持ち続けられるように支援する」「目標を細分化し、ご本人の意向を確認しながら近隣やご家族とできる範囲で進める」等、協力者を得てご本人が孤立しないよう働きかけを行っていた。
