项目摘要

本研究目的は１）臨床応用可能な小型センサと実行機能に関わる課題を用いて、課題中の立位姿勢制御を定量的評価すること 2）実行機能に関わる課題中の立位姿勢制御の測定値について、転倒予測指標として利用可能であるかを検証することであった．2022年度は、分析対象者を若年女性群21名、高齢者群25名、転倒リスク高齢者群12名の3群とした．実行機能の指標であるStroop干渉時間後、①前方の星印を注視する静止立位、②フォームラバー上で前方の星印を注視する静止立位、③フォームラバー上で実行機能課題＋静止立位の3条件において、重心動揺、足関節、膝関節、股関節運動の累積角度を算出し、3群間比較を行った．また、転倒指標であるFunctional Reach Test(FRT)、Timed Up and Go Test(TUG)、実行機能課題中の立位姿勢制御の測定値との相関性について検討した．統計解析の結果、他の2群と比較し、転倒リスク高齢者群の特徴として四肢骨格筋量、FRTの有意な減少、Stroop干渉時間の有意な増加を認めた．立位における下肢関節運動の累積角度は、測定条件①②において足関節の累積角度の群間変動を認めた．測定条件③では他の2群と比較し、転倒リスク高齢者群の股関節前額面、股関節水平面の累積角度で有意に増加する特徴を認めた．また、測定条件③の関節運動の累積角度は、転倒リスクの指標であるFRTやTUG測定値との弱いまたは中等度の相関性を認めた．若年成人と比較し、高齢者の立位では床面の不安定性により足関節運動の累積角度が増加し、床面の不安定性に加えて実行機能課題を実施する場面では、各関節運動の累積角度は増加することを確認した．特に転倒リスクを持つ高齢者では股関節・膝関節運動を増加させ、立位姿勢調節を行う可能性が示唆された．

The measurement condition ③ is to identify the characteristics of intentional increase in the cumulative angle of the femoral joint forehead surface and the femoral joint horizontal plane of the two groups. The cumulative angle of joint motion of the measurement condition ③ is not equal to the cumulative angle of joint motion of the measurement condition ③. The index of joint motion of the measurement condition ③ is equal to the cumulative angle of joint motion of the measurement condition ③. If the age of adults is higher, the cumulative angle of foot joint motion increases, and the cumulative angle of foot joint motion increases. In particular, the possibility of hip joint and knee joint movement increase and posture adjustment increase for those with high posture.
